A group of friends—Haley, Grant, Paxton, Paige, Madeline, Lucas, and Elise—rent a mansion in the Catskills for Elise’s birthday. With tension in the group following Haley and Grant’s recent breakup, they distract themselves by having Haley read their horoscopes using a box of strange tarot cards discovered in the basement. Elise receives The High Priestess card, Lucas The Hermit, Madeline The Hanged Man, Paige The Magician, Paxton The Fool, and Grant The Devil. This leads to an argument between Haley and Grant. Finally, Haley reads her own horoscope and gets the Death card.
The next day, the group returns to campus. Elise is attacked and killed by a monstrous version of The High Priestess. Lucas is killed by The Hermit at a train station. Each death corresponds to the tarot readings, making the group suspect something is wrong with the deck.
They consult Alma Astron, an expert on tarot and the occult. Alma identifies the cards as belonging to an 18th-century astrologer who cursed the deck to kill anyone who used it after her daughter was killed on the orders of a grief-stricken Hungarian Count. Alma urges them to destroy the deck, which is still at the mansion.
While driving to the mansion, their car breaks down, and The Hanged Man kills Madeline. Paxton, terrified, abandons the others and returns to campus, only to be stalked by The Fool. Haley, Grant, and Paige return to the mansion but find they cannot burn the cards, so they seek Alma’s help. Alma attempts to summon the astrologer’s spirit but is killed by the Six of Swords.
Paige is gruesomely sawed in half by The Magician. Desperate, Haley decides to read the astrologer’s horoscope, hoping it will end the curse. As Grant is dragged away by The Devil, Haley reads the astrologer’s horoscope and gives her the Death card. She realizes that all her cards are in reverse and in pain. Haley releases her own grief over her mother’s death, which in turn frees the astrologer’s spirit and destroys the cursed deck.
Haley and Grant reconcile over their survival and reunite with Paxton. Paxton survives after his roommate opens the elevator door at the last second, causing The Fool to disappear. The ordeal leaves them scarred but grateful to be alive.
